UPDATE: Ottumwa woman killed in head-on crash

According to the Jefferson County Sheriff’s Office accident report, Maria Fernandez, 68, was killed. Rebecca Jo Hanna, 54, and Valerie Banks, 47, both of Fairfield, and Maria Martinez, 61, of Ottumwa were injured. Hanna and Banks were passengers in a vehicle driven by Kevin Dale Hanna, 56, also of Fairfield. Kevin Hanna was not injured. Martinez was a passenger in Fernandez’s vehicle.

According to the report, at 5:44 a.m. Friday, Fernandez was driving a green 2003 Honda EX eastbound on Old Highway 34. She lost control of the vehicle just east of the iced-over bridge near the bottom of the hill, crossed the centerline and struck the white 2010 Toyota Venza driven westbound by Kevin Hanna.

The report states that Fernandez was extricated by non-mechanical means and transported by Midwest Ambulance to Jefferson County Health Center, where she died as a result of the crash.

Hanna’s passengers, Rebecca Hanna and Banks, were both extricated by non-mechanical means, and had suspected serious/incapacitating injuries. Rebecca Hanna was transported by the Fairfield Police Department to Jefferson County Health Center, and Banks was transported by Midwest Ambulance to the health center, where they were treated. Martinez also was transported to the health center.

According to the report, the roadway was dark, it was snowing and ice/frost was on the surface of the road.

Both vehicles were totaled.
